COVID-19 COLLEGE CHALLENGE
MTSU has taken pledge and is committed to helping end the pandemic. MTSU Student Health is offering vaccines to all in the campus community age 18 and up. Every shot is one step closer back to the campus life we know and love.

In order to keep Student Health Services informed on the campus community’s status please fill out this online form for students, faculty, or staff who:
- Test positive for Covid-19 OR
- Have symptoms consistent with COVID-19 OR
- Have sought COVID-19 testing OR
- Are close contacts of positive COVID-19 persons
